Proof of Vaccination - Club Policy

The Region of Peel's Medical Officer of Health has issued a Letter of Instruction that requires all people age 12 and older who participate in indoor organized sports, including players, coaches, officials and spectators, to provide proof of vaccination beginning November 1, 2021.

Similar to our current policy on proof of vaccination for spectators, those Participants 12 years and older will need to show their vaccination receipt and proof of identification.

Effective Wednesday September 22, 2021, the Club will implement the following protocols for Proof of Vaccination as legislated in the Reopening Ontario Act, Ontario Regulation 364/20 and Proof of Vaccination Guidance.

  1. Upon arrival at the facility, the parent may enter the facility with their child.
  2. A staff member will ask if the Daily Screening has been completed.
  3. A staff member will ask to see the parent's vaccination receipt, or the participant's (if 12 years of age and older) that shows they are fully vaccinated.
  4. The staff member will need to validate the identification of the vaccine receipt offered by the parent and or Participant to ensure it belongs to them. Validation of vaccination will be based on two key identifiers, note that a photo identification is not required.
    1. Name of the identification holder, and
    2. Date of birth
  5. Proof of identity can be established using documentation issued by an institution or public body, provided it includes the name of the holder and date of birth. Examples of identification documents that may be used to confirm the identity of the holder of the vaccine receipt include:
    1. Birth certificate
    2. Citizenship card
    3. Driver's license
    4. Government (Ontario or other) issued identification card, including health card
    5. Indian Status Card / Indigenous Membership Card
    6. Passport
    7. Permanent resident card
  6. Validation of identification is considered to have been completed / successful when the name and date of birth of the presenter of the vaccine receipt and the name and date of birth on the identification document match. If the name and date of birth on both documents do not match, the individual will not be allowed to remain in the facility.
  7. The staff member will record they have verified the vaccine receipt and identification document for that individual so that on their next visit they will confirm the same individual is spectating in the viewing area or participating in the class.
    1. If a different parent / guardian attends on another date, then they will have to be validated and provide proof of vaccination.
Reminders & Questions
  • A child who is under 12 years of age is not required to show proof of vaccination.

  • What happens when my child turns 12?
    • Participants who turn 12 years of age shall be afforded a grace period of twelve weeks from their birthday to be fully vaccinated before requirements to show proof of vaccination.
  • You must print and bring a copy of your vaccine receipt with you showing that you are fully vaccinated and an identification document. For the purpose of this section, a person is fully vaccinated against COVID-19 if they have received,
    • the full series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines,
    • one or two doses of a COVID-19 vaccine not authorized by Health Canada, followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada or,
    • three doses of a COVID-19 vaccine not authorized by Health Canada; and,
    • they received their final dose of the COVID-19 vaccine at least 14 days before providing proof of being fully vaccinated.

  • It would be extremely helpful to our staff if the same individual attends their child's classes so that we only need to validate one individual per family.

  • If I am participating as a parent in the Parent & Tot class do I have to show proof of vaccination? Yes, anyone over 18 years of age participating in our Parent & Tot class must show proof of vaccination to participate in the class.

  • What if I am not fully vaccinated but I want my child to participate? You can drop your child off with us at the beginning of class, you must then leave the facility and return at the end of the class to pick up your child.

  • What if I have to bring a sibling with me to the facility who is 12 years of age or older; will I have to show proof of vaccination? Yes, youth spectating at a facility used for sports and recreational / competitive fitness activities must show proof of vaccination.
    • However, we have asked that no siblings of any age be brought to the facility to spectate at this time. We understand that this is not always possible, but please try to make alternate arrangements whenever possible for the care of siblings.
